New Health IT Solution Launched by CarePort

CarePort, a health IT company, has introduced a solution for ambulatory providers to improve care coordination. Access to patient records across the care continuum is critical to enhancing care quality as hospitals increasingly rely on mobile providers to provide complete patient care.CarePort Ambulatory allows physician groups and other ambulatory providers to connect patients to home-health services and community-based organizations that provide resources and services like durable medical equipment, physical therapy, transportation, and food insecurity programs through an electronic referral process.

Lissy Hu, MD, company’s CEO and founder, said, “As care is increasingly delivered outside the four walls of a hospital, physician groups and other ambulatory providers require the capabilities to support whole patient care. Our newest offering enables ambulatory providers to efficiently coordinate with home- or community-based organizations, and track and monitor care across settings – helping achieve the best possible outcomes for patients.”

Providers can incorporate the care coordination system into their EHR, allowing them to use the referral function immediately inside their workflow. The application will enable providers to search a national database of services to discover the most excellent match for a patient. Then, providers can maintain two-way communication throughout the referral process to ensure that patients’ requirements are satisfied at home or in the community.

The database, which includes data from over 1,000 hospitals and external data sources such as health information exchanges (HIEs), gives physicians real-time information to track patient discharges, transfers, and readmissions across the continuum. According to advocates, this insight into patient status will allow medical groups to keep in touch with an individual’s health journey and, if necessary, re-engage with a patient. Saint Peter’s Healthcare System recently integrated CarePort in New Jersey to comply with the new Medicaid/Medicare conditions of participation rule.
